Obtain a Sick Note (Med 3)
Doctors Sick/Fit note for absences of more than 7 days
This form can only be used to renew an existing sick note. For new requests, please phone the surgery. Please tell us the dates you need covering and the condition leading to your absence. We require 48h to process the request. You do not require a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.
