Online Services

You can use the NHS app or SystmOne online to view your medical information.

You can create an NHS login without having access to SystmOne online, but some features will be limited.
To get full access and view your medical records both systems need to be linked so you’ll need to request access to SystmOne online services as well. 

If you have already registered for SystmOne online services please click HERE.

There are a number of facilities that York Street patients can access online.

  • Ordering repeat prescriptions
  • Booking appointments
  • Cancelling appointments
  • View your clinical record 

To take advantage of these services you will need to complete  THIS form. This form is only for patients aged 16 years and over. Proxy access is given to parents automatically up until their 11th birthday. Practice policy is that we do not offer online access to patients between the ages of 11-15 years. However applications will be looked at on a case by case basis. For patients aged 11-15 years, parents may apply for access to their child’s medical records if the child consents, has competence capacity, it is not a contradiction to the child’s wishes and it does not go against the child’s best interests. Proxy access forms are not available on our website. Please contact the surgery on 01223 364116 for this or visit the practice in person. 

We will need to confirm your identity before the the service can start. After submitting your form please email the practice on york.street@nhs.net with ID (passport, driving licence or birth certificate).  ID for parents and children will be required for proxy access applications.

To view your clinical record you will need to complete the above application form EVEN IF YOU ARE ALREADY AN ONLINE USER FOR OTHER SERVICES.

If you are booking an appointment online please state the nature of the appointment. Your appointment is not confirmed until you do this and may be cancelled. This is to ensure you have booked with the appropriate clinician.
