Request for Online access to your medical record

Please use this form to request online access to your medical record. You can choose a number of options - appointment booking, order medication, view record. Before this service is made available we will need to confirm your identification.

  • "It’s your choice"

    If you wish to, you can now use the internet to book appointments with a GP, request repeat prescriptions for any medications you take regularly and look at your medical record online. You can also still use the telephone or call in to the surgery for most of these services except ordering repeat prescriptions. Being able to see your record online might help you to manage your medical conditions. It also means that you can even access your record from anywhere in the world should you require medical treatment on holiday. You will be given login details when you first apply. You are responsible for ensuring that only you have access to these. If you choose to share this with a family member or carer, the practice and its staff take no responsibility with what they may see in your record. Access to your record will be given from the date of registration for the service. If you require retrospective access to your record this will take time to organise as we have to remove sensitive and third party information. Given the amount of work involved we can only process a certain number of requests each month. You will therefore be added to a waiting list and notified when access has been given. All applications need to be authorised by your GP. The practice has the right to remove online access to services for anyone that doesn’t use them responsibly. It will be your responsibility to keep your login details and password safe and secure. If you know or suspect that your record has been accessed by someone that you have not agreed should see it, then you should change your password immediately. If you can’t do this for some reason, we recommend that you contact the practice who can arrange for your password to be reset. If you print out any information from your record, it is also your responsibility to keep this secure. If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all. 1:20 relationships can have some element of coercion. A coercive or abusive partner could see if their victim had obtained contraception when asked not to start on this. Or they may have had a termination. They may be keen to note that their victim does not report the true nature of the mechanism of any inflicted injury. This might lead victims to fail to report such issues to their GP. Sensitive information might be in a report from a psychiatrist, the police or in their children's record. They might infer that their victim has disclosed abuse if a consultation has been redacted from the online view. Perpetrators might have access to the NHS App, share the same mobile, know the victim's PIN number or may coerce the victim into sharing their login details. If you think you may be pressured into revealing details from your patient record to someone else against your will, it is best that you do not register for access at this time.

Once you have completed this form we will need to see ID before setting up online services, please either email us a copy of your passport, driving license or birth certificate to york.street@nhs.net or bring this into the surgery.
