This app is designed to help you take control of your asthma.
York Street Practice, working with the Pharmacy next door, aims to improve blood pressure check uptake which is a major health focus. York Street currently conduct around 3,000 checks annually but need to do more.
Blood pressure (BP) readings vary, and repeated monitoring helps produce more reliable results as BP tends to drop outside of the clinic setting.
The age range to be targeted is 45 or over and those who have not had a blood pressure check in the last 5 years, should do.
Our patients receive an invitation to come in for this. Many take this offer up. Many do not.
On occasions when you are in a consultation, the clinician may take your blood pressure. If not please feel at ease to use the one on the left at the Reception Desk, which is sheltered, and hand you’re reading to the Receptionist.
If this is inconvenient, please bear in mind the York Street Pharmacy next door will undertake this for you. The Partners are content with this as we know they will inform you accordingly:
So please feel at ease to get your blood pressure checked here or at the Pharmacy next door.
