In order to register with us we need you to check your eligibility and then complete four online registration forms below. If you are unable to complete the online forms, please contact the surgery. Please note that we cannot register you in person at the practice.

For each child under 16 years old that you wish to register, please only fill in forms 1, 3 and 4.
We encourage everyone to register for online services. This will allow you to book appointments, order repeat prescriptions and view your medical record. In order to have online access to your record, please read the patient information leaflet and complete the online application form below. Once you have completed this form we will need to see ID before setting up online services, please either email us a copy of your passport, driving license or birth certificate to york.street@nhs.net or bring this into the surgery. All applications need to be authorised by your GP.
