If you have already registered for online services please click HERE.

There are a number of facilities that York Street patients can access online.

  • Ordering repeat prescriptions
  • Booking appointments
  • Cancelling appointments
  • View your clinical record 

To take advantage of these services you will need to complete  THIS form. This form is only for patients aged 16 years and over. Proxy access is offered to parents of children up until their 11th birthday, please contact the surgery to request this. Online access is not available to 11-15 year olds. We will need to confirm your identity before the the service can start.

To view your clinical record you will need to complete the above application form EVEN IF YOU ARE ALREADY AN ONLINE USER FOR OTHER SERVICES.

PLEASE NOTE THAT WHEN BOOKING AN APPOINTMENT ONLINE IT IS ESSENTIAL THAT YOU STATE THE NATURE OF YOUR PROBLEM. THIS IS TO ENSURE THAT YOU ARE DIRECTED TO THE MOST APPROPRIATE CLINICIAN. IF THIS IS NOT DONE YOUR APPOINTMENT MAY NOT BE CONFIRMED.
